Substance Use Disorder Health Home (SUDHH)

A “health home” is not a place to live—it’s a model of care that coordinates your health and social needs. You’ll receive enhanced recovery-centered services, as well as a personalized plan for your team of providers. Participation is voluntary.

Recovery Incentive Program

This program funded by the Michigan Department of Health and Human Services (MDHHS) aims to provide contingency management to eligible Medicaid and Healthy Michigan Plan (HMP) beneficiaries. The programs provides motivational incentives to individuals living with substance use disorder for meeting treatment goals.

Enhanced Women Speciality Program

Funded by Michigan Department of Community Health, this Mid-State Health Network (MSHN) women's specialty program provides a unique opportunity specific to parents' treatment needs for substance use or co-occurring disorders. The program directly relates to parents' concerns which include: relationships, physical and sexual abuse, parenting, and skills development for children.

Bridge The Gap

Bridge the GAP is a partnership between MMR Community Paramedics and Recovery Pathways designed to get you treatment as soon as you are ready. We meet you where you are - in the community, at home, or after an emergency visit - and help you begin medication, support, and recovery services right away.

Safety Services

Safety services for substance use disorder focus on minimizing the negative health, social, and legal impacts of drug use rather than requiring abstinence. It includes strategies like providing access to clean syringes, overdose prevention education, naloxone distribution, safe consumption spaces, and medication-assisted treatment. This approach meets people where they are, respects their autonomy, and aims to keep them alive and as healthy as possible while supporting pathways to recovery when they are ready.

We have narcan available on site for clients, friends, family, and the community. This included Narcan dispensing boxes available outside of both Bay City and Corunna locations. We have testing strips, lock boxes, wound care kits, as well as other harm reduction supplies upon request

Substance Use Disorder (SUD) in Youth

15% of high school students have reported use of illicit or injection drugs. These include cocaine, inhalants, heroin, methamphetamines, hallucinogens, or ecstasy. Clients 14 years of age or older can receive mental health services or substance use disorder services without parental consent. This includes up to 12 sessions or 4 months. Staff will maintain confidentiality unless concerns of abuse or neglect are present due to mandated reporting requirements. We assist through therapeutic intervention and medication assisted treatment.

Enhanced & Women's Speciality Service Programming

Duration of the program
- Enhanced Women's Specialty Service Program - 18 to 24 months expandable up to 3 years
- Women's Specialty Service Program - As long as you have legal rights to a child 17 and under
Specific supports available through the programs are peer recovery support, case management, and home-based services.
What are Women’s Specialty Services?

Women’s Specialty Services are programs designed for women who need substance use disorder treatment and are also pregnant and/or parenting. These services address women’s and their children’s physical and behavioral health needs.

In Michigan, pregnant women or women with parental rights to their children are a priority for admission to treatment services. Parents involved with Children’s Protective Services or whose children are in foster care or in placement with relatives are also eligible for services.

What Programs Are Offered?

These specialty services provide or arrange for:
- Primary medical care for women (i.e., prenatal and childcare for dependent children)
- Primary pediatric care, including immunizations for children
- Gender-specific substance abuse treatment and other therapeutic interventions for addressing relationships, sexual and physical abuse, and parenting
- Therapeutic interventions for children in the custody of women in treatment may focus on their developmental needs, issues of sexual and physical abuse, and neglect
- Case management and transportation services
- Childcare for women to attend services
